Circare is designated by the New York State Department of Health as a Lead Health Home. In collaboration with Health Homes of Upstate New York (HHUNY), we manage a network of providers in 10 Central New York counties, who work with individuals with complex chronic health and/or behavioral health needs to help them achieve positive health outcomes.
