Who We Are

History

Circare was founded in 1989 as Onondaga Case Management Services in response to the New York State Office of Mental Health’s initiative to provide intensive case management services to individuals diagnosed with mental illnesses. Over the years, clinical, vocational, peer-based, forensic and crisis-intervention services for youth and their families were added to further support those we serve.

Among its many programs, Circare houses county, state and federally sponsored initiatives including the multidisciplinary Assertive Community Treatment (ACT) team. The ACT team provides 24-hour outreach and services within community settings, such as a person’s own home and neighborhood, local restaurants and parks.

In 2013, Circare received designation from the New York State Department of Health as a Health Home. As a “Lead Health Home,” Circare links Medicaid beneficiaries to care management services, connecting them with care managers located at Circare or other community non-profits. Circare contracts with organizations in Broome, Cayuga, Chemung, Chenango, Cortland, Madison, Onondaga, Oswego, Tioga and Tompkins Counties to provide Health Home care management. Circare is a member of Health Homes of Upstate New York (HHUNY), which is a consortium of four Health Homes.

In 2016, the organization carried out an effort to develop a brand reflective of the organization’s growth, service expansion and the areas it serves in the Central New York region. Onondaga Case Management Services was rebranded and reintroduced as Circare on September 12, 2016.

Circare became the new brand for Onondaga Case Management Services, Inc. in September 2016.

We take Cir from circle to represent the emphasis on the whole person and the idea of creating circles of support for our clients.

CARE

We use the word Care to specifically denote both the practice of care management that is the foundation of our organization as well as the myriad ways in which services are provided today—clinical care, crisis services, vocational, peer-based—and additional services that will evolve in the future.

Our Logo

Our new logo was intentionally designed to represent the notion of a person at the center of a community of care.

The figures in the logo represent a person surrounded by their support system, both personal and professional supports, and the integration of their care.

The circle denotes a focus on the whole person.

The colors symbolize the integration of the various components of a person’s health: physical, behavioral, social.

As a system, these ideas become our Circles of Care℠ that reflect how we approach providing meaningful help for those we serve.

Integrating Health Services signals to the provider community, our community of service recipients, the community as a whole and to our funding sources CIRCARE’s fundamental goal of integrating health services to better support overall well-being.
