ACT Mental Health Team Leader

F/T Assertive Community Treatment (ACT) Team Leader is an individual with a strong background in recovery-based, person centered, behavioral health services to lead an Assertive Community Treatment Team.

Principle Duties and Responsibilities:

  • Provides strong operational and clinical leadership in conjunction with Team Psychiatrist and a multi-disciplinary team
  • Provides direct services to persons enrolled in the ACT Team as needed
  • Responsible for developing and utilizing quality and outcome measures to improve team performance
  • Excellent interpersonal and collaborative skills

Job Requirements:

  • Master’s in Human Services field or licensure registration as RN or Physician
  • LCSW, LMSW, LMHC, or LMFT preferred
  • Excellent computer skills, including significant experience with Excel
  • Must have a valid driver’s license and own transportation
  • Experience on a multi-disciplinary community-based team preferred
