Circare Health Home (HH) Gap in Care (GIC) Peer Engagement Specialist

Circare Health Home is seeking to hire two full time Peer Engagement Specialists (PES) to partner with the Health Coordinator on a Gaps in Care (GIC) project. The PES will engage people in community settings in the 9 County service areas (Broome, Cayuga, Cortland, Chemung, Madison, Onondaga, Oswego, Tioga and Tompkins). The individual in this role will focus on creating a warm hand off between hospital staff and outpatient providers.

Duties and Responsibilities:

  • Provide an encouraging opportunity for individuals in need of treatment for mental illness to engage in dialogue that includes engaging and adhering to treatment as well as information about enrollment in Health Homes
  • Assist individuals in scheduling and keeping 7 and 30 day follow up appointments with Behavioral Health providers post discharge
  • Provide empathy, relationship building, and information sharing
  • Partner effectively with hospital social work departments and other staff to insure effective hand offs
  • Provide linkage to established providers as well as referrals and information regarding new community providers
  • Communicate effectively with individuals and family members who may be ambivalent about pursuing treatment and enrollment in Health Homes
  • Provide ongoing follow up post hospital discharge in order to engage individuals and families in services (e.g. telephonic, community, and home based outreach)
  • Establish and maintains relationships with community provider organizations
  • Effectively document contacts with utilizers of service
  • Collaborate with Health Homes of Upstate New York (HHUNY) GIC Team
  • Complete initial and ongoing required Circare Trainings
  • Complete initial and ongoing required HHUNY GIC Trainings

Education & Experience:

  • Bachelors preferred in health care field
  • Experience/knowledge in healthcare community required
  • Experience working with underserved, transient populations
  • Experience as a peer/ individual with personal experiences of recovery regarding a chronic illness such as a Mental illness, Substance Use or chronic physical illness
  • Have or obtain any one or more state certifications for Peers such as a Certified Peer Specialist (CPS), Certified Addiction Recovery Coach (CARC), or Certified Peer Recovery Advocate (CPRA) at the time of employment, or within one year of commencement of employment 
  • Have knowledge of signs and symptoms of mental illness
  • Have knowledge of local and regional support and recovery resources for mental illness
