Training Coordinator

FT Training Coordinator provides training and technical assistance in support of the Care Management Agencies and Care Managers under the Circare Health Home program.

Principle Duties and Responsibilities:

  • Provides training, consultation, and coaching to support development of agency and program levels of
  • Care Management practice within the NYS Health Home program
  • Works with the Circare Health Home to provide on-board training for new Care Managers and on-site coaching for the Care Management Agencies
  • Uses live and online training curriculum to meet the needs of the workforce within the context of an evolving healthcare environment
  • Maintains solid working relationships with multiple stakeholders including agency/program leaders, supervisors and front-line staff
  • Utilizes media to conduct online meetings, webinars, and conference calls to provide trainings and consultation remotely
  • Stays current on literature and trends in the field in terms of content expertise and best practices for training and implementation and infuses this knowledge into delivery of services
  • Supports the care management record audit process as requested
  • Culturally responsive to the population being served in the development, design, monitoring, evaluation and/or implementation of programs and services


QUALIFICATIONS/SKILLS/EXPERIENCE:

  • Bachelor’s degree in a related field required
  • Knowledgeable about evolving trends in Healthcare Reform
  • One (1) year experience working with care management agencies, consumers and diverse community members preferred
  • Demonstrated experience in human services arena, including training and new program development and implementation
  • Solid computer skills, including word processing, spreadsheet and presentation software required
  • Strong leadership skills including: relationship development, consensus building and facilitation skills
  • Ability to foster collaboration, build consensus and inspire a shared vision
  • Excellent verbal, written and interpersonal communication skills including demonstrated ability to engage with diverse constituencies
