Peer Services

Circare’s Peer Specialists are not traditional mental health providers, social workers or care managers. Rather, they are members of the community who have or have had challenges in life. Peer Specialists partner with individuals in the community who have challenges related to mental health, and support them in their recovery process. They share life experiences with fellow peers to enrich, strengthen, support and create a life of choices. The Peer Specialist builds a supportive relationship with the participant that will foster self-discovery and trust. Peer Specialists can also offer guidance, support, and encouragement in a time of crisis.


Peer specialists and participants work together to promote self-advocacy, choice and empowerment and work collaboratively to identify goals and a plan to achieve those goals. Skill sets and topics outlined in a care plan may include but are not limited to the following:

  • Explore and Connect to Community/Prepare for Social Connection
  • Emotional Health
  • Bus Training
  • Financial Fitness
  • Sustaining Basic Needs
  • Physical Health
  • Effectively Using Paid Supports

Eligibility Criteria/Referral Process

  • Enrolled in Circare’s Health Home Care Management program.

    If you are interested in Peer Services and already enrolled in Health Home
    Care Management, your Circare Care Manager can initiate a referral.)
  • 18 years or older
  • Mental health diagnosis

Hours of Operation

  • Peer Services are available 7 days a week from 8:30 am to 8:30 pm. Participant and Mentor decide together on meeting dates and times.

Circare also offers peer services through Home and Community Based Services (HCBS) program. More information about HCBS is available here.
